passport sent in courier but no VISA. What could that be?

Please let me know if anyone experienced this. After the visa interview the passport was sent in courier (I was assuming VISA is issued and not denied). When I received the passport after three days there is no VISA in the passport. Any comments?

Have you said anything wrong, like any false date or something like that.
My uncle not mention his last refusal and they said you got the visa and they don't when he saw the passport.

Or some one has complaint about you in embassy

I am in the similar situation. I got my passport today in mail but it has no visa stamp. Could you please share your experience with me. Any help or advice will be much appreciated!
