My wife, a British national, has recently received her passport with a stamped immigrant visa from London, UK. Her issuance date was 17JUN2009 and the expiration date was 18May2009 when it should of been six months past the issue date. Is this a typo or does it mean she has been denied a VISA to the USA? If denied a visa, will we be notified why?

That is a typo. Expiration date can not be earlier than the issuance date.
