Surname left blank in passport..

Hi..

My father is planning to visit USA in B2 visitors visa.... But surname is left blank in his passport and full name is entered in Given Name. But the online form for DS-156 is not allowing the surname to be left blank.What shall i do without risk of visa getting rejected? Please help as soon as possible.

Last name = That entire name
First Name = FNU
