questions about my mom's visitor visa

Hi,

I have taken appointment for my mom's visitor visa. I have couple of doubts regarding the documents.

1. As i am on H1 and my husband on h4. Though I am sponsoring..do i need to send my husband's document details like his passport,visa status etc.
2.About mom's letter affidavit ..that affidavit should be done on the plain paper or on stamp paper.
3.About My mom&dad 's marriage certificate....that a marriage certifate is must or affidavit saying that they married on so and so date and their name.
They did affidavit as they married many years ago..and could not able to get a certificate now.

Please help me on these above questions asap..will really appreciate.
