At the end of the sample 'affidavit of father' on immihelp.com, it is written 'Deponent sworn to before me'? What does it mean exactly? Who needs to sign the affidavit? Does it has to be on any legal paper as 'stamp paper'?

Yes, it has to be on stamp paper.

Your father needs to sign in front of notary and then notary has to sign.
