Previous Visa Rejections !!!

My Father in Law is planning to apply for US visa to visit us, he had two Visa refusals in previous expired passport (not the one he has now) long back, one in 1996 and other 1999.

does he need to mention these very old refusals in DS 156 ?

does he need to carry previous passports ?

Any help/guidance will be appreciated.

Yes. He must mention them. The consular officers KNOW this already, Why try to lie? and get banned for life.
