  • DS-156 Form

    Hello!

    I already have an appointment for interview with the US Embassy, the problem is when I'm reviewing my Form DS-156, I noticed I haven't completed some fields by indicating None or Not Applicable. Instead, I just left them blank.

    Do I have to make another form? If so, does it mean I have to make another application? and my appointment now will be put into waste?

    This is my first time, i hope you could help me. Thank you

    If you have not filled it according to that, you will have to do it all over again.

      Don't worry about those blanks. When you fill in 'None' or 'NA' on the DS156 form and then download it for printing those sections just come out as blank.

      So you don't need to re-do the form.

        They ask you to fill it None or NA so that someone does not add something to the form at a later date that you did not intend to add. I have seen verification officers at consulates ask the applicant to write NA in those with a pen before they are sent to the interview room/section.
