Arranging Documents

Hi.. I'm planning to bring my parents to US. I'd some doubts regarding the manner in wich they should arrange their documents:

1) do they need to place a covering letter or is the affidavit serves the purpose ?
2) Are the DDs to be placed along with the application form or separately?

There is no specific order to place the documents in.
There is no drop box. It does not matter how you keep demand draft with respect to application form.
