Need clarification on I134 info

I need a clarification on the date info in the below item in I134.

2. I am _ years of age and have resided in the United States since(date) -----

I came to US first on 28 Sep 2000 in B1 visa and then went back to India in Dec 2000. In Feb 2001 came back in H1 and stayed for 1 1/2 years and went back to India. After two years (in June 2004), I came back to US in H1 again and am in US since then.

So in my case, what should be the date for the above question.
Please clarify.

in that line something like

residing since June 2004 [visited in 2000,2001-2002]
