Form I-134 Doubt Passport Place of issue

My father's passport was issued in Dubai in 2001 for one year and it expired on 2002. Before the passport expired in 2002 my father applied for a new passport in chennai but got the same passport extended in chennai till 2011. My doubt is on the form DS156 for Passport place of issue what do i have to put in? The passport number is the same because rather than issuing a new passport they gave an extension on the old passport itself. Will the VO have any issue regarding this.

Is it the same booklet or they gave you new one?

If it is the same, write Dubai.
If he got new booklet, write Chennai.
