launguage problem

i am doctor, gone for visitor visa on bases of conference attending on USA. i took my wife along with me at VO. my wife had problem with english language spoken by VO. she not understood. even i made a mistake, as understanding question,as they spkeak fast in american english. routinely i haven't any problem with english in medical field. they rejected us as we not understood & answer properly within short time.
next time if i choose gujarati language in interview, have any negetive effect in getting visitor visa, as i am in medical field & want to go for conference in usa.
