DS - 158 : Is it required ?

HI,
I took an apptmt over web for my parents. In the instruction form which we got after taking the apptmt, it says there shud be a DS-158 for all applicants. Is this true ? I was under the impression that it is required only for male applicants between the age of 16 and 45.
Please clarify.
