I-134 couple of questions

1. I am on H1-B, Do I have to give an 'A' number or I leave all the 4 options of item 1 blank.
2. On item 2, On how long have I resided? Do I put in the date when I first came to US (4 yrs ago) or the last time I entered US(last year).
3. I donot have W2 form for year 2000, but I do have the copy of tax filings. What can I do about it? I have all the forms for 2001, 2002, and 2003.
4. Do I have to send the original offer letter or a recent company letter describing my position and status.

Your answers would be highly appreciated.

1. I am not sure - as I did not come via H1-B

2. Length - time when you first entered - 4 years.

3. Honestly it is your payslips for last 4 -6 months are good enough. if you want to show W2- please do, but don't worry about 2000 - W2.

4. You want your company to give a letter describing your position and status...
