very confused in filling b1/b2 visa form(ds156/157)

Me and my husband want to apply for usa b1/b2 visa,can u plz letme know about follwing questions:
1.my husband don"t use any surname as per passport his given name is s kumar and sirname is blank.so,how can we fill surname in ds156 visa application form for tourist.

2.i am very confused about question no:24 and 25.in Q24 i will fill hotel name & address(newyork) and what about question no:25 which is "Name and telephone number of erson in u.s.Who will be staying with or visiting for tourism or business...? i dnt know any one in U.S.A so what i have to fill.OUr plan is after visa approval we will buy individual holiday package from any tour operator.
so plz help me i am very confused..?

1. Write that entire given name in surname field. In first (given) name field, write FNU.

2. Write None.
