documents needed for a 3+year old

Hi,
my sister and her 3+year old boy are planning for a US trip.I'm in US on H1B visa and sending all the required documents for my sister.I would like to know that all the docs are required for her 3+year old kid also or just a separate DS-156 form will be enough for him?

Just DS-156 and I-134 for child.
