Applying for US visit visa (B2)

I want to visit US for holiday in April/May; Currently I am working in UK in Power Industry. My questions are

• What documents do I need to submit, I know some of the documents like completed application form, passport, photo. Etc (after browsing through your website) What letter should I need to get from my employer? Do I need my holiday approval letter? Do I need to show hotel booking and itenary? How much money I should keep in the bank account and how long do I need to keep in account?
• Do I need to book flight ticket as well?

Could you please help me by answering these questions and any other documents I need to prepare apart from the above

Currently I am working in UK and I have friend (he is just a student) in USA who I want to visit and would like to go for site seeing in USA as well for 2 weeks. I have got answers for most of my questions from the link which you sent me in your email. I have some more questions which I can’t find it on the link. Could you please tell me?

• Whether I can apply for visitor visa from UK itself How many months pay slip do I need to show how much money I need to show in my account and for how many months I need to keep the minimum balance (if possible please tell me the amount in pounds)
• Do I need to get any sorts of letter from my friend who is in USA
