Questions on I 94 number and date

We are preparing documents for our parents visa. This site has been of immense help. I have a few queries though.

In I 134, in the 1st item, it asks for the I 94 number and the 2nd item asks for the date since which we are residing in US.

Actually we came to US in June 2006, went to India in April 2008 and again came to US in July 2009 and are here since then. Which I 94 number and date should we mention in those columns?

Amd also is it possible to fill the DS 160 online , take the print out and send to our parents for signature. They do not have proper internet access. Or is the electronical signature mandatory?

Thank you administrator for the clarification. But the date since which we are in US, should it be the first time entry's date? Because the Bank Letter will be showing all the earlier dates too. Or should we mention the date on which we entered for the second time?

There is no clear rule on that. You can do it either way.
