I am a US Citizen and need bilateral lungs transplant. As a requirement for lungs transplant by UCSF, I need two caretakers. My wife is one but I don’t have second care taker. So I wanted my nephew from India to come to USA on visit visa to take care of me for my transplant. My Dr wrote medical certificate for my need and my nephew as one of the two required care taker. However electronic form has no place to attach medical documents or the travel agent in India did not know. So US consulate did not even see my sponsorship form and medical certificate when my nephew went for visa interview even after his requesting them to see the documents. I think consular officer has a
ready makeup their mind not to give him visa before he went for interview. I went to immigration attorney in San Francisco and he said they don’t do visit visa. He is 45, married with wife with 3 kids in India 2 of them still in India. So she has to take care of his family in India and is going to go back after my transplant. So I was thinking of applying for him d onetime using old style paper form (not electronic) so that I can attach relevant documents. Does anyone have pdf or word format Application form for visitor visa for me to fill out and send to US consulate in Bombay? I can’t find it on UCSIS website. Or is there any attorney who can help me with this?
