Old Passport details, in DS 160

Hello All,

My father in law had old passport (issued 20 years ago) which got expired and he misplaced it after he got his new passport 10 years ago. Now, While filling DS 160 for my Father in Law, Do we need to mention that?

Below question from DS 160 is this for old expired passports.?

DS 160 Question

Have you ever lost a passport or had one stolen?

If we state 'Yes' for above question, What are supporting documents and options we have as we don't have any formal police complaint as it got expired 10 years ago and he got new passport?

Old Passport was never used to travel and doesn't have any visa's on it.

New passport has mention of old passport number and details, Do they ask details on it and can my father in law state the fact that its damaged? Do we need to provide any proof stating that its damaged?. Adding to that name in old passport is different compared new one. Like below;
Old Passport Name: Last Name: Kasi, First Name: Ram Vishwanan
New Passport Name: Last Name: Kasi, First Name: Ram Vishwa Rao
