About DS-156 form

I was filling the form DS-156,in Passport section what should I write in Surname as in passport they did not mention surname, all given in "given names".
Second I don not have address to fill in the blanks,where to stay in USA as I decided to book hotel after receiving Visa, so what should I write in that box.

1. You should be filing DS-160.

2. Yes, you can put some hotel address.
