My cousin requested to come into the U.S. for 3 months but he received a no AOS/EOS/COS on his B2 visa. Can we ask for an extension? If so how does this work? It is his first time coming into the U.S.

Sometime for visitors they put this remark, so that they won't be able to switch to any other visa or extend their stay. You can still file EOS & stay, but if his extension is denied then he might get trouble in future visits/visas.
My suggestion is to have him go back before I94 expiry for his futures entries in USA without any issue.- I am not an Attorney, hence not giving any legal advice. Just sharing MY opinion with an intent to help others.
