DS 160 - employer information - for B2 visa

I was filling Ds-160 for my mother.

1) My mother is a retired teacher from a school run by Govt. of Kerala. so, what should I enter for Employer name address city state tel# ??
2) supervisor's name, employment date
she retired about 10 years back .

all of thes info is needed for B2 Visa ?

how about for the question - previous employment ?

They answer previous employment in last 5 years. Therefore, you don't have to mention something 10 years ago.
