Passport track

My parents appeared at new Delhi on 20 dec 2019, VO said issued.
Father's passport tracking status earlier was " still with embassy" but today changed to " no record available". And visa check status is refused.
Mother's passport was ready for pickup for 1 day. And from then it is " has been returned to post".

Why 2 different status
