B1/b2 visa rejected-- have to appear for usmle exam in may--- please help!

My b1/b2 got rejected on 17/1/2020 because my purpose was to go to USA for observership and exam.
I had a visa letter from an agency which said it was a clerkship but clerkships are done by students on J1 visa which I had not known.
Now I want to apply again for 2 weeks just for the exam.
Should I apply just with exam documents or should I arrange for an invitation letter from a distant family friend?
Please guide me.
My exam is on MAY 12, 2020.
