Two different names for my father

I got weird situation here , please advice me .
The first name for my father in my passport and his passport are little different . He worked for a central bank and got retired now . The name in the bank records where he gets pension and my passport names are same . But his passport and his property i n home town are in different name . How can i approach in order to get B2 visa for my parents. Our surnames are same . A minor change at the end of the name is differ like " ramaiah " in my passport and his passport as " ramana reddy " .
your advice is greatly appreciated . I paid my fees and looking for slots

Last name changes

Thanks for the reply ! I started getting affadavit for the same .

During the affadavit process i came to know that the documents submitted to get passport is only one set of documents like ration card, pass book which govt issued for lands and lawyer affidavit . No where it was mentioned that he was a retired Bank employee.
Now if we go through the affdavit will there be any issue as we show the both evidences for 2 names like bank pension , tax return forms etc vs ration card , passbook ? Do embassy have access to check what has been submitted along with passport application.

Please let me know what's the best thing i can do at this time not to get into any sort of trouble.

Does this affidavit requires notary attestation or magistrate attestation or self declaration as per link is enough ? I have seen some cases where they applied for name change by giving advertisements in papers ..let me know which one to proceed. Please reply . Thanks in Advance .
