Clarifications on DS-160 form for visitor visa

Hello

This is Kiran. I have got an appointment and have filled-in DS-160 form. I am looking for clarifications on couple of questions here

1. DS-160 online application form should be filled-in only for the primary applicant or this should be filled-in in 2 sets (one for the primary applicant & the other for the dependent). Please note that I have filled-in the details for my wife when I was filling-in and had selected an option where I was asked "whether am accompanied with anyone on entry to US" (I selected spouse). Please let me know should I fill-in another application separately for my spouse now?

2. On my confirmation letter of US visa appointment, I see "Prepaid, self-addressed express mailer, 1 per family". Would like to know what is expected here

I have filled-in a separate DS-160 application for my spouse. And I have added my spouse as my dependent when scheduling for the appointment. So the same appointment works for both of use right. Please confirm
