Ds-160 from

Hi, pls can anyone help me, I started filling my F-student U.S Ds-160 form. After creating security question, I filled my surname which is a four letters word and year of birth, i clicks on Save, it logged me out and requested for the 'first five letters of my Surname' when the surname I filled in was a four lettered word. How may I do , retrieval is denied.
