Reapplying for mom: PLEASE HELP !!!

HELLO THERE
last december my mom and dad got rejected under 214 b rule. now my mom is going alone for visa on 17th feb. I am on H 4 and my husband is the main sponsor. my Q is:
1)should my husband send a new employment letter which mentions only my mom or the last one with both my parents in it will do????
2) she will be applying in mumbai, so does he have to send his birth certificate??
3) Can i write the invitation letter to my mom or it has to be written by my husband only????

Do we need to mention parent's name in employment varification letter?

In the sample E.V. letter on this site, there is no mention of person who is sponsered. Is it really required? From Bijal's post it seems like her husband had included her parent's name in the letter.

Also, I do not have my original birth certificate as my birth was not registered. I do have affadavits made two years ago which I had used for my green card for counsular processing at Mumbai counsulate. Can I use those same affadavits?
