confusion about filling up of DS 156 TOURIST VISA APPLICATION FORMVISA

  • confusion about filling up of DS 156 TOURIST VISA APPLICATION FORMVISA

    I have some confusion about filling up items 7,8, & 9 of the B2 tourist visa application form DS 156, for my brother in law. My brother in law`s name stated in all his official records, except his passport, is S. P. MANOJ.

    In his passport, his name is stated with the expansion of his initials S.P, viz. SAROVAR PRAKASH MANOJ { which is written in his passport as,
    surname - SAROVAR , and given name -PRAKASH MANOJ} .

    His family name is SAROVAR , His father`s name is PRAKASH, and
    His own name is MANOJ. so kindly clarify the following:-

    (1) how to write his FIRST and MIDDLE name as in PASSPORT, in item 7
    of the DS 156 Visa application form ?. as MANOJ PRAKASH or as
    PRAKASH MANOJ ?


    (2) in item 6 of DS 156 form, his surname is written as SAROVAR,as in his
    passport, wether he should fill up in item 8 ( other surnames used )as
    S P,and item 9 (other first and midlle name used ), as MANOJ,
    since his name is stated as S.P. MANOJ or MANOJ S P, in all other
    official records, like income tax returns, tax receipts, bank a/cs education
    certficates etc, OR just write NIL , in items 8 & 9.

    if my above 2 queries are not clear to you, kindly let me know, how to fill up, items 6, 7, 8 & 9 of the DS 156 application, for the above person whose same name is wrtten in 2 different manner, in his official records and in his passport as stated above.

      Requesting Specific Reply To My Queries

      I have read the contents of the LINK provided by you in your reply to my above queries.
      but it does`nt give a clear answer to my queries. so kindly give me a specific reply to my queries (1) & (2).

      in the GIVEN NAME of passport, the FATHER`Sname is written FIRST,
      and HIS OWN NAME is stated as SECOND name. so if in item 7 of DS 156,if the GIVEN NAME(2nd line of the passport name ), is wrtten in the SAME ORDER, father`s name will become the FIRST name, and his own name will become the MIDDLE name (in item 7 of DS 156). I thInk his own name should be his FIRST name & his father`s name should be his MIDDLE name, in item 7 of DS 156. hence my QUERY NO. (1)., and the WEB LINK given by you , do not. give a clear answer to the above.

      similarly 2 SURNAMES are used, in DIFFERENT records, of the same person, dueing the same period.( SAROVARAM and S P). hence the query no.(2), the WEB LINK do not give a clear answer to this query also.

      I understand that many VISA applicants have the above two queries / confusions. so I request you to kindly give a specific reply to the above 2 QUERIES, which will be very helpful to sevarel FORUM members, and readers. Thanks a lot.

        Own name == Given name == First name
        Father name == Husband name == Middle name
        Surname == Last name

          Thank you very much for the clear advise given for my query no.(1)
          But I think your advise/ reply do not cover my query no.(2), about filling up
          items 8 &9 ,of DS 156 TOURIST VISA application.
          so kindly advise for my Query no. (2) also. Thanks.

            Using S.P. is not another surname. It is just an abbreviation.

            Therefore, no need to put anything.
