My wife is a Dutch national and holds a Dutch passport. She also has UK nationality but no British passport. She has always been known in the UK by her married name but her Dutch passport is in her maiden name followed by her married name. That is how the Dutch write their passports for married women, no exceptions. Should the application be in her married as she has always been known by that for 40 years, or her name as in the Dutch passport ?

When submitting the application, it's best to use the name that your wife has always been known by in the UK for the past 40 years. In this case, her married name should be used for consistency and to avoid any potential confusion. While her Dutch passport may list her maiden name followed by her married name, using her married name in the application aligns with her established identity in the UK.
To ensure a smooth application process, it's essential to provide consistent and accurate information, reflecting the name she commonly uses in the UK. This will help avoid any potential discrepancies or complications during the application review.
If you have any specific concerns or need further guidance, I recommend reaching out to the relevant authorities or seeking professional advice to ensure a successful application.
