Citizenship referees: what information?

I’m getting ready to apply for citizenship and will need to approach my proposed referees this month.

I’m wondering what information I require from them for the purposes of my (online) application form (e.g. is it name and email address, or more than that?), and where to obtain the current version of the (paper) form(s) that the referees need to complete?
