Hello all,
I am filling the DS 160 for my mother's B2 visa. Could you please help me answer the question "Have you ever used other names (i.e., maiden, religious, professional, alias, etc.)?". I am aware that I need to answer this question as YES since there has been a name change. But, how to answer 'Other Surnames Used' in this case, since old passport surname is empty.
Additionally, the instructions for this section - Other Names Used - is "If you only have other surnames to enter, enter the same given names as above. Conversely, if you only have other given names to enter, enter the same surname as above."
Old passport (cancelled):
Surname - EMPTY (no surname)
Given Name - Shalini Lal
New passport:
Surname : Patel
Given Name - Shalini
Since the field 'Other Surname used' cannot be left empty, please advise how to write this information? Thank you so much in advance.
