Visa for Mom. Please Help.

Hi,

My Mom is applying for US vistor's visa and my husband is sponsoring her.

My Questions are

1) My mother's name in my passport and her passport differ by a single letter.
Will it affect us from getting her visa?

2) My Mother dont have any assets in her name and everything is in the name of my dad and he is not applying for visa and he will be staying here with his parents.
What my Mother has to mention for this..

What should be the minimum balance that the sponsor(My husband is sponsoring my mother and i am an house wife) should have in his US account.
We got the bank letter from BOA regarding our bank details but it is not signed and it simply looks like a plain document with no attestation.
Please reply me whether it should be got attested.
In what situation we need to submit the birth certificate.
