Reinstatement of visitor visa

If an extension of stay in the US is denied, my understanding is that your 10 year multiple visa is automatically is cancelled. If the visitor wants to visit the US again can they simply get their visa reinstated (and if so how) or does it mean that they have to apply for a new visa just like anybody yes. If they live outside of Chennai does it mean they have to apply personally for an interview.
