Urgent:I 94 missing How to obtain...

My self and my wife travelled to US on B2 visa Dec 2010.We have applied for extension and suddenly found I 94 for my wife is missing while planning to send the relative documents.Of further examination it is found that the date stamp( permission to stay up to) in my wife's pass port is also missing. We have return tickets for 25th June.Please advise how to get the I 94 duplicate issued and get extention for stay with out problem.

You need to apply for duplicate I-94's, but you will not get those before June 25. It is best that you leave the US as planned and after leaving send copies of your boarding passes to London, KY address from CBP website.
