Affidivit of assurance

I would like to ask anyone who can provide a sample affidivit of assurance to come back to home land..
I do not have much idea how to wirte it.. even i searched the site but that looked to me for parents. i want to write it for my self...WOuld any one help!or can anyone provide me a sample!
