Visa - Form Query

Hi

I would like to know what do i need to write in the form for Who is paying for your trip ?

Should i write myself or my uncles name as he has send me a sponsor letter.

Also is it compulsory to mention my maiden name in the form since now i am married and have a differnet surname.



Please let me know asap

Who is actually paying for the trip? Write that, and show appropriate proof.

Write your current name as in the passport, and write maiden name in other names used.
