Below are few questions regarding filling the ds forms.

q. 25. tel no and name of person in us ……
as I’m going to attend a scientific conference hence would be staying at a hotel.
Now whose details shud I specify here? The hotel details or the conference
Details.
q.28 . who will pay for the trip?
Myself and parents will be paying for the trip hence shud I write
Myself, parents or myself, father, mother??
Q 40. application prepared by?
I’ll be myself preparing the application hence do I have to leave this column
blank or write none????

q 3 . name in native alphabet??
Full name in english is required???
Q8 full name and address of contact person /organization???
Shud I write the hotel details or the conference details??the organizers??
Q13. List all professional , social and charitable organisations ……
Shud I specify the institute I’m working with or the affiliations like the
medical councils , other societies etc.
q17 listing the educational institutions (not elementary)
shud I mention only the graduate college or the details of Xth nd XII are also
required??
Q 18 specific travel arrangements….
I’ve booked airlines tickets , hotel reservations .. but the space is limited if I
Type on the form . Can I type the rest of the form and write in ink in this
Column. Can I write attached itinerary in the column and supplement the
details in the separate sheet of paper.?
Please respond appropriately and at the earliest I’m scheduled for the interview in couple of days time.
