My MIL was granted a VISA 5 yrs ago & due to some personal problems she did not visit US. Now, She wil be appearing for an Interview at the Consulate with my FIL also.

My Q is : Since she had not make a trip previously, Will this pose any concerns / issues that we need to be aware of ?

That is no problem. It does not matter that she did not visit.

If they ask her that question about ( I think they definitely will )..

' Why she did not make that trip '? She does not have a convincing answer, she just didn't make it due to some domestic problems. Any thoughts ????

It is her wish that she did not make the trip.

It is not mandatory to travel within X duration after getting visa. With 10 years visa, person can travel at any time within 10 years.
