Visa Invitation

I'm trying to have my Mom visit me for two months. I'm a naturalized US citizen. Have a great job and good income.
So far I know I have to fill in the I-134 form. Here is my questions. Who do I send it to? what are the things that my mother has to do in terms of her paperwork?
