name different in different document

Hi
I want to apply for tourist visa for US.
My passport has only my first name. The same is the case with 10th class certificate- birth date proof
To show income proof, I would require bank statements, job proof and Pan card.
all these documents have my surname on it. will it be a problem in applying for the tourist Visa.

should i mention the my surname in the space provided for alias/other names

also, would i require any affadavit at the time of interview.

If you have last name, you should mention it.

Otherwise, they will put your first name as your last name, and first name would be FNU.

thank u for this info...but my question is that my financial proof which will show that i ve strong ties to india and that i have a job has a surname which is nt present in my passport.how will they accept it...do i need to make a affadevit saying that all these names are for only 1 person n should nt be considered as diferent ppl...
