Visitor visa (B2) for my parents and my nephew

Hi,

I'm on H1B visa and working in USA. I would like to bring my parents
and my nephew (my sister's son who is 9 years old) to USA for 3 months
on Tourist visa (B2). I will be sponsoring all 3 of them (my parents and nephew).
My sister and her husband do not have visa and Im not applying visa for them.

I'm going to send the following docs

1) copy of my visa, I-94, passport
2) copy of last 3 years W2
3) copy of my bank statements, bank account verification letter
4) copy of last 3 months pay stub
5) copy of my birth certificate
6) employment verification letter and business cards
7) I-134 for each of them

My parents are going to present
1) Own house document
2) no objection letter from my elder brother in India
3) family photos

And for my nephew, in addition to the above 7 docs, he will be presenting

1) no objection letter from his parents to send him with my parents to USA
2) letter from school to allow him to go to USA and come back before start of next year class (June).


Now my questions are

1) Do my sister and her husband need to accompany my nephew for the visa interview?
2) Because my nephew is not going to come with his parents instead his grandparents, will that
be an issue during the interview?
3) Are there any other documents we should be presenting for this case?
4) Is it mandatory to apply visa for my sister and her husband if we are to apply visa for their son?
5) Will I be required to prove relationship with my nephew (or) is it required for my parents to prove
relationship with their grand son. If so, what are the docs?

I got one more question.

I'm planning to send I-134 for each of my parents and nephew.

In the form for my father I can give his information and mention
the accompanying person as his wife (my mother). But there is
no option to select 'Grand son' in that table. So, how do I mention
that my nephew is accompanying them.

Also in the form for my nephew, how do i mention that his grand parents are accompanying him?
