No family name

Strangely enough for you probably, I don't have a family name. In my country, it's not common for people to have a family name unless you belong to some tribes. But I don't belong to any so I don't have it. Would it be a problem if I leave the Surname column on DS 156 empty?

That's OK. You can leave blank or write 'LNU' (Which stands for Last Name Unknown).
