Birth certificate

I do not have my birth certificate and hence trying to get affidavit done from my parents. I found sample at the link below as affidavit-mother and affidavit-father, but can I change it to one affidavit which has both mother's and father's name in it instead of 2 separate affidavits.
Do we have a sample for that.
