Trouble adding family member to appointment schedule for US B1/B2 Visa

Hi there,
I am the principal applicant applying for B1 visa to US from canada. I have filled out the DS-160 online form and got DS-confirmation number for myself, and my spouse and my two kids as my family members. I was able to add my spouse to schedule an appointment for visa interview in the same time slot of the same date. However when i try to add my kids i get an error saying "error creating applicant." I have entered all information correctly and followed the step by step protocol. What might be causing this error.
