Name messed up in passport, ticket and visa

I need help. Again.

My father is travelling from India to USA next week. I just realized that his name in passport, ticket and visa is a bit messed up (last name and first name merged together and became his first name). However the mess is consistent. His name, although wrong, is printed as same in all three. Should I be concerned about it? Can there be any issues during the travel? Do you have guys have any suggestions?

That's fine. Most airline tickets show up as all names merged anyway.
