Im a H1-B1 employee.
For Question 1, as per immihelp guideline, im going to enter

e. H1-B1 (Temporary Worker) - <a number>


Question: What is < a number>?
Is it the Control # as seen in the VISA/I-94 or
is it the Receipt number as seen in the H1 B1 affidavit or some other number that is expected.

The receipt number has hypens in betwen them ( ex: SO-098-UHD-78978)

A number stands for Alien registration number on your Green card (only applicable for US permanent residents). If you are on H1B, you do not have an A number.

Perhaps an H1B approval I-797 receipt number... Note that the original USCIS I-134 form did not accomodate the non citizen / non USPR categories and hence IMMIHELP is showing a way to provide that information in the sample form...
