Question on I-134 Affidavit of Support

Hi,

I am sponsoring my Sister and her family( Brother in law and Nephew) for B2Visitor visa. When trying to fill I-134 form , the instruction said that 'separate I-134 form needs to be sent for each alien' , does this mean

1) I should send 1 form for my sister , 1 for my brother-in-law and 1 for my nephew?
2) if so, what should be filled in for question #3 'This affidavit is executed on behalf of the following person' in each of the forms.
