I am on B2 visa for 6m,I94 expires on Nov 22nd 2012.I want to apply for extension of stay for further 6months to help my daughter who is undergoing training in medicine .What reason I should give while applying for extension.

1.I had been visiting US on B2 visa several times ,once I stayed for 6 months ,twice for 4 months and twice for one month ie my stay was limited for one month twice.This time I came here after 8 months stay in India. I have been given 6m at POE .My I 94 date is 28th Nov 2012. If I apply for extension of stay will it be granted.
2.If I am denied will my visa become void even if I return before 28th November. Can I visit US after 6 months stay in India with the same visa if my request
for extension of stay is denied.
3.What should be filled in item 11 of I 134, should tick "intend"?.If so whether "room and board "or "money " .if "money" how much dollars.
should attach salary certificate, bank statement and previous years' IT return ? What other documents are needed.
4.If I am granted extension ,if not needed and I return to India before 28th Nov 2012,can I apply for extension when I visit US after one year.
