Visitor's visa extension clarifications reg

I am in US for 6 months from June 2012 ,I- 94 valid till Nov 28th,planning to apply for extension of stay
reason being : to help daughter who is in medical training ; is stressed.
My questions are:
1.Will the above reason accepted by USCIS,
2.I have been in US for 4m Oct -Jan 09;6months Mar10 to Aug 2010;
4m from Feb2011 to June 2011;twice my stay was limited to one month ie in Jan 2010 & Sep 2011
Will I be given extension
3.While applying ,in item 11 of I 134 what should be filled in ,"intend" wheter board & room or money,if money how much.
4Whether copy of existing return ticket on 22nd nov should be sent
5 If I am denied extension,will my visa become void even if I leave US before I-94 date
6 Can I enter US again in the same visa after 6 m ,even if I am denied extension

Most probably not. 99% of college students are stressed while they're in college. It is a extremely weak reason.

Nobody can answer that question. But read (1) before.

Extension of stay is I-539. And you can file online.

You should read Form M-752, Helpful Filing Tips for Form I-539.

Sigh. General rule is "spend outside the US two times the time you were in the US". So if your current stay is 4 months, you should be outside the US for *at least* 8 months. Look at your own travel history - in two occasions, you were given only 30 days.

Don't push your luck. If your visa is cancelled because of your travel patterns, it will be extremely difficult for you to get a new one.
